Korea’s smallest preemie goes home

One of the most premature babies ever born in Korea, whose survival was doubted, goes home today after nine months at Samsung Medical Center.

Kim Eun-shik is now a healthy, 3.57 kilogram (7.9 pound) baby. He was born on July 12, 2010, after only 25 weeks of gestation, weighing just 280 grams.

His 40-year-old mother, surnamed Lee, suffered a pregnancy disorder and gave birth prematurely.

Doctors expect him to grow normally and suffer no health issues as a result of being born prematurely.

“An extreme preterm baby grows well without major difficulties if he survives,” said Dr. Park Won-soon of Samsung’s pediatric department. “Saving a baby with a birth weight of less than 300 grams is a feat that surpassed the former limits of modern medical science.”

Eun-shik’s lungs weren’t fully developed at birth because the lungs are one of the last organs to develop in the womb. Doctors injected a pulmonary surfactant to help Eun-shik’s lung inflate and not stick together.

Eun-shik underwent heart surgery just three days after birth and had a hernia repair surgery a few months later.

Before Eun-shik, a baby girl born in 2004 after 26 weeks of gestation and weighing 434 grams was the most extreme preterm baby in Korea. The girl survived and is now in elementary school.

Eun-shik’s parents spent around 30 million won ($27,521) for their son’s nine-month treatment. The National Health Insurance Corporation covers expenses for incubator use for premature babies, which costs 18,000 won per day.

An American baby was born in Seoul a few years ago at 22 weeks of gestation. The world’s most premature surviving baby was a girl born at 21 weeks and six days of gestation in Miami, Florida, in 2006, who weighed 280 grams and measured 24 centimeters.

380g 은식이의 기적

최경량 미숙아, 278일 만에 집으로

국내에서 가장 작게 태어난 아기가 생후 9개월 만에 귀가를 앞두고 있다.

성균관대 의대 삼성서울병원 소아청소년과 박원순·장윤실 교수팀은 14일 “지난해 7월 12일 국내 최경량아로 출생한 김은식군이 16일께 건강한 상태로 퇴원한다”고 말했다. 어머니 이모(40)씨의 심한 임신중독증 때문에 김군은 예정일보다 4개월여 빨리 세상과 만났다. 출생 체중은 초미숙아의 생존 한계로 간주하는 400g(임신 24주 미만)에도 미달했다. 정상보다 15주나 빠른 임신 25주째에 태어나면서 체중은 정상(평균 3.2㎏)의 10분의 1보다 조금 무거운 380g이었다. 지금은 건강하게 자라 체중이 3.6㎏으로 불어났다.

초미숙아는 폐가 발달하지 않아 출생 후 숨을 제대로 쉬기도 어렵다. 출생 당시 김군은 눈을 거의 뜨지 못했고 폐 발달 여부를 보여주는 젖꼭지도 보일락 말락 했다. 박 교수팀은 오그라든 폐를 펴기 위해 계면활성제를 투여하고 아기가 분당 900회가량 호흡할 수 있도록 돕는 고빈도 인공호흡기를 사용했다. 김군은 생후 3일 만에 심장수술을 받은 데 이어 입원 도중 장이 막히는 바람에 탈장 복원수술을 받는 등 두 차례 큰 수술을 극복했다.

박 교수는 “초미숙아는 초기에 생사의 고비만 넘기면 별 문제 없이 잘 자란다”고 말했다. 장 교수는 “체중 300g대의 아기를 건강하게 잘 살려낸 것은 현대의학의 생존한계를 극복한 것”이라고 설명했다. 그간 국내 최경량 초미숙아는 2004년 삼성서울병원에서 태어난 김소망(26주+4일, 434g)양으로 올해 초등학교에 입학했다. 김군 가족이 지난 9개월간 지불한 입원비와 보험 처리가 안 되는 검사료(본인부담액)는 모두 3000만원가량으로 추산된다. 미숙아의 인큐베이터 사용료(하루 1만8000원)는 건강보험공단에서 전액 지원한다.
