Climate change brings new disease to Seoul

Climate change is not only bringing warmer, wetter weather but also subtropical diseases rarely found in the northern part of the country, including scrub typhus - a mite-borne infectious disease caused by a parasitic micro-organism.

Once only found in the southern end of the peninsula, typically affecting farmers in the area, scrub typhus has now become a common illness all the way up in the southern region of Gyeonggi and is continuing to move northward into Seoul.

Also known as tsutsugamushi disease, scrub typhus last year affected 536 Gyeonggi residents, a sharp spike from just 131 cases in the province in 2001.

This disease, which can be fatal, is transmitted by mites, and symptoms include a fever, headache, rash and swollen glands.

In one case from last year, a 74-year-old woman surnamed Kim in Gwanak District, southwestern Seoul, went to a health center after suffering from a fever, lack of appetite and severe muscle pain for 10 days. Doctors there diagnosed her with scrub typhus.

On her chest was a scratch that looked like a mite bite, and it was later confirmed that Kim had been bitten last September while she was taking a stroll near Mount Gwanak.

It wasn’t only Kim who was shocked by the diagnosis but also the medical team at the health center, as the disease was thought to be found in subtropical areas.

Indeed, Seoul is no longer an exception to scrub typhus. From 2001 to 2010, the number of cases in Seoul increased from 126 to 187.

Park Mi-yeon, director of communicable diseases between humans and animals at the Korea Centers for Disease Control and Prevention, said that temperatures in Seoul have risen enough for micro-organisms to propagate.

According to health authorities, the higher the temperature during the summer, the more people get infected by scrub typhus during the following fall season.

“It is very likely for the number of people infected with tsutsugamushi disease to increase this coming fall as the temperature this month as well as humidity in June and July were quite high,” said Professor Jang Jae-yeon, from the department of preventive medicine and public health at Ajou University Hospital.

Jang also said that high temperatures can trigger various allergies, including hay fever, as the blooming season has advanced and become longer, increasing pollen in the air.

After analyzing 2,000 patients who suffer from various allergies from 1999 to 2008, Jang said he found that there was a close relationship between temperature and an increase in the number of patients.

“When there was an increase in the minimum temperature in March, there was an increase in the number of allergy patients such as with asthma and rhinitis during April and May,” said Jang.

아열대 쓰쓰가무시병 ‘서울 습격’

기후변화가 질병 지도 바꾼다 - 온난화 탓에 쓰쓰가무시병, 천식, 비브리오 패혈증 환자 급증

이명박 대통령은 8일 라디오 연설에서 “기후변화가 당초 예상한 것보다 훨씬 빠르게 전 지구를 위협하고 있다”며 “삶의 방식에 대한 대전환을 요구받고 있다”고 말했다. 반기문 유엔 사무총장도 11일 방송기자 토론회에서 “기후변화 문제 해결이 나에게 주어진 중요한 과제”라고 밝혔다. 기후변화의 주원인은 지구온난화다. 지구 기온은 과거 100년간 0.6∼0.7도 오른 것으로 알려져 있다. 기후변화는 생태계에 큰 영향을 미친다. 동식물 분포도를 바꾸고 나아가 질병의 발생 양태를 바꾼다. 벌써 이런 영향이 나타나기 시작했다. 

서울 관악구 김모(75) 할머니는 지난해 10월 엿새간 열이 나며 식욕이 없고 심한 근육통이 생겨 인근 보건소를 찾았다. 보건소에서 쓰쓰가무시병 진단을 받았다. 가슴엔 진드기에 물린 듯한 상처가 나 있었다. 지난해 9월 서울쪽 관악산 자락에서 진드기에 물린 것이다. 보건소 직원들은 크게 당황했다. 쓰쓰가무시병은 아열대 지방의 대표적 질병. 국내에선 남부지방에서 종종 발생한다. 서울에 출몰한 것은 드문 일이다.

 쓰쓰가무시병이 북상하고 있다. 이 병은 털진드기가 옮기는 병이다. 가을걷이를 하던 남부지방의 농부들이 많이 걸렸다. 최근 몇 년 사이에는 경기도 남부지역에도 흔한 병이 됐다. 이 병에 걸린 경기도 환자가 2001년 131명에서 지난해엔 536명으로 급증했다.

 서울도 예외가 아니다. 같은 기간 서울 환자는 126명에서 187명으로 증가했다. 하지만 서울 환자는 경기도와 좀 다르다. 서울 환자 대부분은 경기도 이남에서 야외 활동을 하다 걸린 사람들로 추정된다. 김 할머니처럼 서울에서 걸린 경우는 드물었는데 요새는 심심치 않게 발생하는 것이다. 이미 서울에도 쓰쓰가무시병이 상륙한 것이다.

 이 병이 고위도 지역에서 가을·겨울에 발생한 적은 별로 없었다. 온도가 낮아 털진드기가 살 수 없기 때문이다. 질병관리본부 박미연 인수공통감염과장은 “서울 관악산에서 한 교수가 털진드기에 물린 적이 있다”고 말했다. 서울에도 털진드기가 살 수 있을 만큼 기온이 올라갔다는 얘기다. 강원도의 쓰쓰가무시병 환자도 2001년 44명에서 지난해 61명으로 늘었다. 이 중 일부는 강원도에서 걸렸을 것으로 추정된다. 질병본부 박 과장은 “재작년 가을 철원의 유치원생이 야외 나들이를 다녀온 뒤 감염된 적이 있다”고 말했다.

 여름에 고온·다습하면 가을에 쓰쓰가무시병 환자가 늘어난다. 아주대 의대 장재연(예방의학) 교수는 “한반도의 8월 기온과 6~7월 습도(장마·수해 등)가 올라가면서 올가을 환자가 늘어날 가능성이 있다”고 말했다.

 여름철에 잦은 비브리오 패혈증도 기후변화와 관련 있다. 바닷물 온도가 올라가거나 염도가 떨어지면 비브리오 패혈증균이 증가한다. 이 병에 걸린 환자는 2001년 41명(24명 사망)에서 지난해 73명(31명 사망)으로 늘었다. 올해도 이미 13명이 걸려 6명이 숨졌다. 이 병은 비브리오 패혈증 세균에 오염된 해산물을 날로 먹거나 이 균에 오염된 바닷물이 상처에 들어가면 걸린다. 봄철과 7월 강수량이 많을수록 환자가 증가한다.

 기후변화는 천식·비염 등 알레르기 질환 발생 위험도 증가시킨다. 기온이 올라가면 나무에서 꽃이 피는 시기가 빨라지고 길어져 꽃가루 양이 늘어난다. 또 꽃가루 안의 단백질(알레르기를 일으키는 항원) 독성이 강해진다. 아주대 장재연 교수가 1999∼2008년 2000여 명의 환자를 분석한 결과 기온과 환자 변화가 거의 일치했다. 장 교수는 “3월 최저기온이 올라가면 4~5월에 천식·비염 등 알레르기 환자가 증가했다”고 말했다.

 기온이 상승하면 세균 번식이 증가해 설사 환자가 늘어난다. 보건사회연구원 채수미 전문연구원과 고려대 의대 윤석준(예방의학) 교수 분석에 따르면 한반도 연평균 기온이 2008년 8.2에서 30년 후에는 8.5도로, 50년 후에는 9.6도로 상승한다. 이에 따라 설사 환자가 30년 후에는 0.9~2.4%, 50년 후에는 4.2~11.2% 늘어나 의료비가 60억~760억원 증가할 것으로 전망됐다.

 폭염으로 인한 사망·열사병 환자 등도 기후변화의 희생자들이다. 장 교수팀이 94년 한반도 대폭염 영향을 분석한 결과 당뇨병·심장병·호흡기 질환 환자의 사망률이 30~43.8% 증가한 것으로 나타났다.

 보건사회연구원 신호성 박사는 “70년대 사라진 말라리아가 93년 휴전선 근처에서 재발한 이후 매년 환자가 증가하고 있는데 이 역시 기후변화 때문인 것으로 볼 수 있다”고 말했다.

◆쓰쓰가무시병=가을철에 주로 유행하는 열성 전염병. 대개 풀숲의 털진드기에 물린 뒤 감염된다. 진드기가 문 자리에 지름 1㎝가량의 붉은 반점이 생긴다. 야외에서 풀밭에 앉지 말고 긴옷을 입어 진드기 등에 물리지 않는 것이 효과적인 예방법이다.
