Fundamental solutions for MERS

There have been no new patients and deaths related to the Middle East respiratory syndrome (MERS) for the last 14 days. Samsung Medical Center, the last hospital under the health authority’s quarantine, returned to normal operations on Monday after a 40-day partial shutdown. At the current rate, the authority would be able to announce the end of the battle with MERS that claimed 36 lives and caused billions of dollars in losses. The outbreak exposed various problems with the national quarantine system, medical services and civilian awareness. The country is vulnerable to another outbreak or epidemic if it does not make fixes.

But from what we have seen so far, we can hardly shake off concerns. The Ministry of Health and Welfare that had promised improvements to quarantine policy and measures has thus far fallen short of expectations. It plans to keep the current Korean Centers of Disease Control and Prevention intact and upgrade its head to a vice ministerial level. The presidential office is considering institutionalizing a vice minister in charge of health affairs. Overall, the actions are no more than feigned gestures.

Frustrated civilian experts issued a statement criticizing the ministry’s makeshift measure of trying to wrap up the case by installing one extra division at the Korean Centers of Disease Control and Prevention.

“Public health and wellness cannot be protected from future infectious diseases with such measures,” the statement said.

They proposed that the center become an independent administration because it can’t function under the existing bureaucracy and command.

The ruling party and presidential office should draw a more farsighted and broad blueprint to reorganize the health and welfare ministry. The two are not related and are too big to share one roof. The health ministry should be separated to oversee student, worker, environment, community and social health care more systematically. Public clinics also should be able to function better under its umbrella. The welfare ministry should be more responsible for the welfare of those with low incomes. 중동호흡기증구훈(MERS·메르스) 신규 확진자가 14일째 나오지 않고 있다. 추가 사망자도 8일째 나오지 않고 있다. 20일을 기해 삼성서울병원이 부분 폐쇄에서 풀린다. 모든 상황이 안정 국면에 접어들었다. 이대로 가면 다음달 하순에 메르스 종식 선언이 나오게 된다. 메르스는 수 조원의 손실을 초래했고 36명의 목숨을 앗아갔다. 방역시스템, 후진적 의료체계 등의 문제점을 적나라하게 드러냈다. 이번에 이런 걸 제대로 손보지 않으면 제 2의 메르스를 피하기 힘들 것이다.
그런데도 후속대책이 영 미덥지 못하다. 책임선상에 있는 보건복지부가 키를 쥐고 방역체계 개선책을 만들고 있다는데, 들리는 얘기를 종합하면 지금과 크게 다를 바 없는 모양이다. 질병관리본부를 그대로 둔 채 일부 조직을 늘리고 질병관리본부장을 1급으로 존속시키려는 것 같다. 다른 한편에서는 청와대가 복지부에 복수차관을 두는 안을 검토하고 있다는 얘기도 들린다. 복지부의 짜깁기 대책보다는 청와대 움직임이 더 나은 듯 하지만 오십보백보처럼 보인다.
이런 상황을 참다 못한 전문가들이 19일 성명을 냈다. 대한예방의학회와 한국역학회는 “보건복지부는 기존 질병관리본부에 위기대응센터 하나 신설하는 것으로 메르스 후속 대책을 마무리하려고 하고 있다. 이 같은 미봉책으로는 미래의 전염병의 위협으로부터 국민의 건강과 생명을 보호할 수 없다”고 지적했다. 질병관리본부를 질병관리청으로 격상해 독립적·전문적인 질병관리조직으로 바꾸자고 제안한다. 행정관료들의 통제를 받는 수동적인 질병관리본부로는 전문성을 발휘하기 어렵다고 지적한다.
새누리당과 청와대는 차제에 보건복지부 개편을 비롯해 좀 더 큰 그림을 그렸으면 한다. 연관성이 별로 없는 복지와 보건이 둘 다 너무 덩치가 커졌다. 보건을 떼내 학생·근로자·환경·지역 사회 보건을 총괄하는 보건부 설립이 필요하다. 일선 보건소도 여기에 넣어 손발이 되게 해야 한다. 복지부의 저소득층 복지와 고용노동부의 일자리 복지를 합쳐 선진국형 복지체제로 가면 좋다. 당·정·청은 전문가의 비판에 귀를 더 열어야 한다.
