Stay on the alert

The steps the government has been making to upgrade health care and the medical system to pre-empt the spread and outbreak of infectious diseases so far raise doubts about their effectiveness. According to its outline to bolster prevention, the Ministry of Health and Welfare plans to escalate the status of the Centers for Disease Control and Prevention to a vice-ministerial office and allow sovereignty in appointments and budgeting. The ministry wants to make the organization as independent and powerful as the U.S. Centers for Disease Control and Prevention (CDC).

But the details of the outline suggest otherwise. The ministry made it clear that the center would be under its auspices. The CDC cannot have full authority over its manpower and spending if it is under the ministry’s umbrella. Quarantine authority and management should be the primary function of the disease center.

For the agency to draw experts or groom expertise, its staff must be able to commit themselves to the work for a long time. To do so, the center should be upgraded to an independent office with autonomous authority in appointment and budgeting. Guising its spin-off won’t make any difference.

Some other proposals are good. Patients must have a treatment request in order to transfer to the family medicine departments in large hospitals, and rates for non-emergency patients should be raised in order to ease emergency room congestion, which has been blamed for the spread of Middle East respiratory syndrome (MERS). But there must be other medical facilities that open 24 hours for patients to turn to instead of emergency rooms.

The government must make fundamental - not makeshift - changes to medical services to prevent future epidemics. The ministry should break up and separate the administration of health and welfare. Private cooperation is also important in preventing and fighting diseases.

Korea’s unique hospital traditions - frequent hospital visits, family care and unbridled use of emergency rooms - all need to change. Visitors are already crowding back into hospital rooms, although it has not been long since the country battled with MERS. Food and flower deliveries to hospitals are also unregulated.

Quarantine efforts relating to MERS became lax after the government announced the disease is no longer a threat last month. But prevention and control of infections must be carried out every day, not just during outbreaks. Hospitals must enforce strict rules on the timing and number of hospital visits. Family members also must cooperate for the sake of their own health and society’s.

보건복지부, 기득권 꽉 움켜쥐고
면회객, 입원실 맘대로 들락날락
선진 의료혁신 기회 놓쳐선 안 돼
중동호흡기증후군(MERS·메르스) 확산 사태의 재발을 막을 정부의 의료시스템 개혁작업이 영 미덥지 못하다. 보건복지부가 18일 공청회에서 밝힌 개편안에 따르면 방역을 맡은 질병관리본부(이하 질본)를 차관급 조직으로 격상하고, 자율적인 인사·예산권을 부여할 방침이다. 복지부는 이번 개편안대로 하면 미국 질병통제센터(CDC)처럼 전문가 권한이 강화돼 지금처럼 질본이 복지부 등의 눈치를 보지 않게 될 것이라고 주장한다.
하지만 이번 개편안을 꼼꼼히 살펴보면 복지부의 조직 이기주의에서 크게 벗어나지 못한 것으로 평가할 수밖에 없다. 질본을 복지부 산하에서 독립시킬 수 없다는 입장을 분명히 했기 때문이다. 조직 자체를 복지부 산하에 계속 둔다면 인사·예산권을 독자적으로 행사하기가 쉽지 않은 것이 자명한 사실이다. 사실 방역체계 개혁의 핵심은 질본 혁신에 달렸다고 해도 지나친 말이 아니다. 그동안 전문가들은 인재를 불러모으고 장기 근무하게 하면서 전문성을 키우려면 질본을 질병관리청이나 질병관리처로 독립시켜 독자적인 인사와 예산권을 부여해야 한다고 주장해 왔다. 그런데 독립에 반대하며 어물쩍 넘어가려고 하는 것은 미봉책일 수밖에 없다.
그래도 진료의뢰서 없이 상급종합병원(대형 대학병원) 가정의학과로 직행하지 못하게 하고, 대형병원 응급실에 가는 비응급환자의 부담을 높이는 등 메르스 확산에 원인을 제공했던 응급실 과밀화 방지대책을 내놓은 것은 평가할 만하다. 다만 야간에 응급실 대신 갈 수 있는 의료기관을 충분히 늘리지 않고 문턱만 높이면 환자 접근성을 떨어뜨린다는 지적에는 귀를 기울여야 한다.
정부는 제2의 메르스 사태를 막기 위해 전염병 대책만을 위한 국소적인 개편보다 의료전달체계의 근본적인 개혁에 눈을 돌려야 한다. 전염병 확산 같은 후진적인 의료사고를 방지하려면 결국 보건행정의 전문성 강화와 의료 선진화가 최적의 처방이기 때문이다. 이를 위해 보건복지부를 복지부와 보건부로 분리해 전문성을 높이는 방안도 적극 검토해야 한다.
제2의 메르스 사태를 막기 위한 민간의 협조도 절실하다. 병원감염에 무방비인 병문안, 가족간병, 응급실 출입 등 세계보건기구(WHO)가 지적했던 의료문화의 문제점이 다시 고개를 들고 있어서다. 아직 메르스 사태가 공식 종료되지 않았는데도 면회객들이 입원실을 마음대로 들락거리고 병실에 꽃과 음식이 무절제하게 배달되고 있다. 메르스가 한창이던 지난 6~7월 문병객은 물론 출입자까지 통제하던 병원의 방역체계가 느슨해진 것이다.
황교안 국무총리가 지난달 28일 사실상의 메르스 종식선언을 한 뒤로 사회의 방역기강이 해이해졌다고 볼 수밖에 없다. 방역수칙 준수는 전염병이 기승을 부릴 때뿐 아니라 평상시에 더욱 필요하다. 그래야 후진형 전염병의 재발을 방지할 수 있다. 병원은 면회시간을 합리적으로 조정해 꼭 필요한 소수의 인원에게만 허락하도록 규정을 바로잡아야 한다. 환자 가족은 이를 따라야 한다. 우리 모두의 건강과 안전을 위해서다. 이대로 방치하면 제2, 제3의 메르스 사태를 막을 수 없을 것이다.
