Hospitals are usually tense, unforgiving spaces with a stern atmosphere that can at times be detrimental to patient care.

But Severance Hospital’s main branch in Sinchon has a warm, welcoming atmosphere that has more in common with a hotel or cultural institution than a medical facility. Light floods into the building through an atrium that extends from the sixth floor down to the basement. The expansive lobby houses a gallery, bank, bakery and restaurants.

Architects who worked on the building’s 2005 renovation were told to create a “hospital that doesn’t look like a hospital building.”

Severance is part of the Yonsei University Health System, which, as the nation’s first Western-style hospital, is the foundation of modern medical treatment and medical studies in Korea.

The hospital was founded in 1885 as Gwanghyewon and was located in Jae-dong, central Seoul, where the Constitutional Court used to be. Since then, it has changed names and locations several times.

Just two years after its founding, it had changed its name to Jejungwon and moved to Gurigae at Euljiro.

It gained the name Severance Hospital in 1904, when it moved again to a new location in front of Seoul Station. It moved to its current location in Sinchon in 1955.

Gwanghyewon’s founder was an American missionary named Dr. Horace N. Allen.

He outlined his belief in the need for a formal hospital in Korea in a request for permission to build the facility that he submitted to King Gojong on Jan. 27, 1885.

“Every city in America has more than one hospital,” he wrote in the request. “There should be at least one hospital in Seoul and it can be built with little cost. Your majesty will find pleasure in seeing people in agony be cured.”

The proposal was accepted and Gwanghyewon opened in April 1885.

Allen’s successor was another missionary named Oliver R. Avison.

The English-born Avison immigrated to Canada in his youth and became a medical professor at Toronto University. He was inspired to pursue missionary work in Korea after learning about the work of Presbyterian missionary Horace Grant Underwood.

When his Canadian Methodist Church said it was not interested in his mission project, however, Avison contacted the Presbyterian Church and they helped him come to Korea.

Avison arrived here with knowledge of modern medical practices, but when he saw the state of Jejungwon (Gwanghyewon’s new name) he decided to try to improve the facility. He secured $45,000 from benefactor L. H. Severance and turned it into Severance Hospital.

Severance once told Avison that his pleasure in giving to the hospital was “bigger than that of the recipient” and his generosity continued after his death. After Severance passed away, his son, John Severance, donated an additional $120,000 to the hospital.

His generosity has inspired others. When Severance Hospital decided to rebuild its main building in 2005, a total of 6,000 people donated more than 20 percent of the total 266.5 billion won ($246.4 million) budget for the building’s construction.

This spirit of charitable giving is underlined by Park Hyung-woo, 55, the director of Dong-Eun Medical Museum at Severance’s Sinchon branch and the author of the book “The History of Western Medical Education.”

“Modern missionary projects in Korea were largely based on medical services and education,” Park said.

“Joseon Dynasty rulers protected missionary activities and our medical center is the foundation of the Korean Protestant church,” he said.

Severance has an important place in Korean history beyond its influence on medicine.

The first seven medical licenses in Korean history were all graduates of Severance Hospital Medical School on June 3, 1908: Hong Jong-eun, Kim Pil-soon, Hong Seok-hoo, Park Seo-yang, Kim Hui-yeong, Ju Hyun-cheuk and Shin Chang-hee. The seven historic figures would later become important to the construction of modern Korea.

“When Korea was annexed by Japan, Dr. Kim Pil-soon was exiled to China and participated in the independence movement,” Park said. “Blood pathology researcher Hyun Bong-hak, the 34th licensee, is called the Korean Schindler or the Korean Moses, because he helped 98,000 civilians escape through Heungnam port.”

The gallery is currently preparing for a photography exhibition that opens this month called “Seoul in the late Joseon Dynasty through [Horace] Allen’s eyes.”

“3분 아닌 30분 진료 시대로 가야, 무상의료 얘기 답답한 일”

최초라는 말은 대개 자긍심과 통한다. 오랜 역사성을 지닌 채 선두다툼을 하면서 여전히 성공적인 경영을 해갈 경우엔 권위까지 붙는다. 연세의료원은 우리나라 근대식 의료와 의학연구의 발상지다. 국가가 세운 한국 최초의 서양식 병원인 광혜원의 맥을 잇고 있기 때문이다.

1885년 서울 재동 헌법재판소 자리에서 출발한 광혜원은 곧 제중원으로 이름이 바뀌었다. 이어 1887년 을지로 입구의 구리개(銅峴), 1904년 서울역 앞 복숭아골 신축 세브란스병원을 거쳐 1955년 지금의 연세의료원 자리에 터를 잡았다. 이후 연세의료원은 대부분의 다른 근대 시설물이 그랬던 것처럼 발전을 거듭해왔다. 강남·용인 세브란스병원에 이어 인천 송도국제병원이 세워질 예정이다.

신촌 세브란스병원 본관은 하나의 세련된 도시다. 의료시설이라기보다 근린시설이라는 느낌을 준다. 로비인 3층의 4000㎡(약 1200평) 편의시설은 호텔·백화점을 연상시킨다. 아담한 전시공간, 은행, 베이커리, 편의점, 전문 식당가 등을 갖췄다. 지하 2층, 지상 6층 높이에 100m 길이의 아트리움(Atrium·중앙정원)은 건물 깊숙이 자연채광을 끌어들인다. 쾌적하고 편리한 공간 구성으로 외래 진료실과 중앙 진료실을 자연스럽게 나눠 동선을 줄였다. 20층에는 전망 좋은 스카이라운지도 있다.

하지만 의료원의 중심축에 우뚝 서 있는 이 본관 건물은 산자락에 동서로 길게 흘러내리듯 자리잡은 병원시설들을 단절시킨다. 그래서 보완책으로 만든 것이 6층 높이까지 뻥 뚫린 중앙대계단(Grand Stair)이다. 3층 출입구와 아트리움 사이에 놓인 계단 위쪽으로 하늘이 보인다. 이 열린 공간은 상징 그 이상이다. 대형 건물이 차단시킨 동쪽 고지대와 서쪽 저지대를 연결해주는 효과가 있다. 저층부 옥상정원은 의료진이 틈틈이 바람을 쐬며 휴식을 취할 수 있는 공간이다. 말안장 모양의 안산(鞍山) 자락 녹지가 가슴에 안긴다.

계단에 앉아서 의료원 정문으로 뻗은 내리막길을 바라본다. 봄날 4월이면 벚꽃이 만발하여 향기를 뿜어낸다. 무더운 여름에는 계단 왼편으로 시원한 물줄기가 흘러내린다. 영락없는 도심의 자투리 공원풍경이 아닌가.

우환을 떨쳐낼 수 없는 게 병원이다. 병원은 최첨단 진료설비를 갖춘 하이테크 빌딩이다. 딱딱하고 차가운 느낌을 줄 수밖에 없다. 병원을 찾는 사람들은 으레 그런 위압감을 감수해왔다. 서양식 병원이 들어오고 거의 100여 년간 그래왔다. 그런 위압감이 정서적으로 환자치료에 도움이 안 되는 건 물론이다. 근래 들어 병원건물들이 변하고 있다. 세브란스병원 본관은 처음부터 건축주와 설계자가 ‘병원 같지 않은 병원’을 짓기로 합의했다. 이른바 사용자들의 뜻을 반영한 참여설계다. 기획설계 단계부터 한국과 미국의 두 회사가 아이디어를 내놓고 보완하며 공동으로 설계했다.

“미국의 모든 도시에는 하나 이상의 병원이 있습니다. 서울에도 병원은 하나 꼭 있어야 하고 그 병원은 적은 비용으로 만들 수 있습니다.… 고통 속에 있는 국민이 적절하게 치료받는 것을 보는 기쁨을 폐하께 안겨드릴 것입니다.” 1885년 1월 27일 고종에게 올린 의료선교사 알렌의 병원설립안은 이내 수용되었다. 제중원은 그해 4월에 문을 연다. 세브란스병원은 이때를 병원 설립 원년으로 삼고 있다.

알렌의 뒤를 이은 에비슨(O. R. Avison: 1860~1956)은 제중원을 세브란스병원으로 키워낸 인물이다. 영국 태생인 그는 유년시절 캐나다로 이민해 토론토대학 의과대학 교수가 된다. 언더우드의 영향을 받은 그는 한국 의료선교를 원했다. 캐나다 감리교회가 한국에서의 사역에 관심을 보이지 않자 에비슨은 장로회와 접촉한다. 감리교도인 그가 교파가 다름을 말하자, 뉴욕 장로회 총무 엘린우드는 “교파가 중요하지 않다”며 “감리회의 열정으로 한국 선교에 열중하라”고 주문한다. 에비슨은 스스로 장로회로 교적을 바꾸고 한국에 온다. 교파를 초월한 감동적인 이야기다.

제중원에서 의료와 의학교육에 힘쓰던 그는 한계에 부딪힌 제중원을 본다. 그리하여 독지가 세브란스(L. H. Severance)로부터 4만5000달러를 기부받아 세브란스병원을 세운다. 현재 가치로는 1000억원에 해당하는 거액이다. ‘받는 당신보다 주는 나의 기쁨이 더 큽니다’. 세브란스가 에비슨에게 한 말이었다. 아들 존 세브란스도 아버지의 유언을 받들어 병원에 12만 달러 이상을 기부했고 사후에도 펀드를 만들어 기부를 계속해왔다고 한다.

세브란스병원은 기부문화가 만들어낸 결실이다. 본관을 새로 지을 때도 6000명이 2665억원의 건축비 가운데 20% 이상을 기부했다. 이 때문에 병원에 사회적인 책무도 따르게 된다. 다른 병원들과 달리 부정적인 면이 조금만 드러나도 상대적으로 더 크게 부각되는 것도 그래서다. 외적인 성장도 필요하지만 그와 동시에 어려운 이들을 조건 없이 돕는 세브란스의 정신을 계승해나가는 것도 중요해 보인다.

연세대학교 의료원은 2005년 복강경수술용 로봇 ‘다빈치’를 국내에 처음 도입한 이후, 지난해 말 수술건수 5000건을 돌파했다. 일본·미국·싱가포르 등의 의료선진국에서도 로봇수술법을 배우기 위해 찾아오고 있다. 하지만 부속품을 미국에서 수입해 와야 하고 건강보험 적용이 안 돼 환자비용부담이 크다고 한다. 국산화가 시급하다.

“한국 근대 선교의 특징은 의료와 교육에 있어요. 그래서 조선 정부로부터 거부감 없이 보호받을 수 있었던 겁니다. 우리 의료원은 한국 기독교의 본산입니다. 그리고 의료면허 1번부터 7번까지가 1908년 6월 3일 세브란스병원의학교를 졸업한 졸업생들이지요. 홍종은, 김필순, 홍석후, 박서양, 김희영, 주현측, 신창희가 성적 순으로 면허를 받았습니다. 김필순 선생은 한·일 강제병합이 되자 중국으로 망명해 독립운동가가 됩니다. 혈액병리학자 현봉학(34회) 동문도 한국의 쉰들러, 한국의 모세로 불립니다. 한국전쟁 때 민간인 9만8000명을 흥남부두를 통해 탈출하게 한 숨은 공로자지요.” 한국근대 서양의학교육사를 쓴 박형우(55) 동은 의학박물관장은 역사적인 인물이 된 동문들 자랑에 바쁘다. 근대유물을 다량 확보하고 있는 박물관은 오는 4월 3층 로비 아트스페이스에서 ‘알렌이 본 구한말 서울’ 사진전을 열 계획이며 ‘알렌·헤론·에비슨·분쉬 유물전’도 기획하고 있다.
