Study: Heavy drinkers, smokers more depressed

The more you drink alcohol and smoke tobacco, the more likely you are to be depressed. That was the finding of an extensive study conducted by the Korea Centers for Disease Control and Prevention (KCDC).

The 2009 survey of 7,480 adults found a correlation between heavy drinking and smoking. The odds of suffering from depression rose 2 to 15 percent for heavy drinkers and smokers. The correlation was even more apparent for women.

The survey found that 33 percent of female smokers suffered from depression, whereas only 18.7 percent of nonsmokers suffered from the same condition. Eleven percent of male smokers experienced depression, compared to 9.7 percent of nonsmokers.

For purposes of the survey, the KCDC defined depression as life-disrupting feelings of despair that lasted for more than two weeks out of the year. The analysis is based on data from the Korean National Health and Nutrition Examination Survey in 2009.

“Smoking might help in relieving depression at an early stage of the disease,” said Kim Yun-ah, a KCDC researcher. “However, when it becomes a habit and extends over a long period of time, [smoking] makes people psychologically unstable in that they rely on smoking when they feel depressed.”

For men, there was a clear correlation between depression and heavy drinking. According to the survey, 9.7 percent of moderate drinkers were sufferers of depression, but that rose to 19.3 percent for those that identified themselves as heavy drinkers. That compares to 7.3 percent of nondrinkers who suffered from depression.

The KCDC researcher said that people suffering from depression tended to depend on either drinking or smoking to relieve stress instead of getting to the root of their problems.

The study also found a correlation between body mass index (BMI) and depression, especially in women. Thirty percent of underweight females, defined as under a “healthy” BMI reading of 18.5, suffered from depression, while about 20 percent of overweight and healthy women were depressed. BMI is a measure of body fat based on height and weight.

“Thin people have a higher possibility of getting depressed because they are more sensitive when compared to healthy and overweight people,” said Woo Jong-min, a psychiatrist at Inje University Paik Hospital. “Depression could make them even thinner by hurting their appetite.”

Woo recommended that depression sufferers use prescription medicine to deal with depression, rather than chain smoking and binge drinking.

공기업 직원 박영호(37·가명)씨는 직장 회식 때 평소보다 말이 많아진다. 이런 모습 때문에 주변 동료들은 그를 쾌활한 사람으로 생각한다. 그러나 집에 오면 내성적인 모습으로 돌아간다. 허전함과 우울증을 못 이겨 거의 매일 술을 마신다. 보다 못한 부인이 박씨에게 병원 치료를 강권했다. 항우울제(抗憂鬱劑)를 처방받아 3개월 정도 복용했더니 우울증 증세가 거의 사라졌다. 마음이 안정되면서 술 먹고 싶은 충동이 크게 줄었다.

 술을 많이 마시거나 담배를 많이 피울수록, 마른 체형일수록 우울증을 더 느낀다는 조사 결과가 나왔다. 질병관리본부는 2009년 국민건강영양조사 결과를 토대로 우울증 요인을 분석해 주간 ‘건강과 질병’ 최근호에 발표했다.

질병관리본부는 전국 성인 남녀 7480명에게 과거 1년 동안 2주 이상 일상생활에 지장이 있을 정도로 슬픔이나 절망감을 느낀 적이 있는지를 물었다.

 조사 결과 술·담배와 우울증의 연관성이 뚜렷했다. 술의 양이나 횟수가 많아지면서 우울증을 느낀 경험률도 올라갔다. 남성 비음주자의 7.3%만 우울증을 경험한 반면 평균적인 음주자는 9.8%, 문제 음주자(과음)는 7.7%, 알코올을 남용하는 사람은 10%, 알코올 의존증이 있는 사람의 19.3%가 그런 경험을 했다.

 남성 흡연자 중 우울증을 경험한 사람이 11.4%로 비흡연자 9.7%에 비해 높았다. 여성은 담배와 우울증의 상관관계가 더 뚜렷했다. 비흡연자의 18.7%, 흡연자의 33%가 우울증 경험자로 나타났다.

 질병관리본부 건강영양조사과 김윤아 연구관은 “담배를 배우는 초기에는 흡연이 우울증을 줄이는 데 도움이 될지 몰라도 습관이 되고 장기화되면 짜증을 많이 내는 등 정신적으로 좋지 않은 영향을 받게 된다”고 말했다.

우울증이 있는 사람은 자신의 건강을 관리하려는 의지가 부족해 운동을 덜하고 흡연이나 음주에 기대게 된다는 것이다.

 비만인 사람보다 마른 사람의 우울증 경험률이 높았다. 특히 여성이 그렇다. 적정 체중인 사람의 19.3%, 비만인 사람의 20.3%만 우울증을 경험한 반면 마른 사람은 30%에 달한다. 남자도 비만(10.6%)에 비해 마른 체형(16.8%)이 높았다.

 인제대 의대 서울백병원 우종민(신경정신과) 교수는 “마른 사람이 통통한 사람보다 상대적으로 신경질적이고 성격이 까다롭고 예민한 편이라 우울증 경험률이 높은 것 같다”며 “우울증으로 식욕이 줄고 그래서 마른 체형이 될 수도 있다”고 말했다.

 이혼이나 사별(死別)로 배우자가 없거나 별거 중인 사람의 27.8%가 우울증을 경험한 반면 배우자가 있거나(13.3%)과 결혼한 적이 없는 사람(13.5%)은 훨씬 적었다. 여성의 19.8%가 우울증 경험자인 반면 남성은 절반 정도(10.1%)였다. 또 소득이 낮거나 건강상태가 좋지 않을수록, 나이가 많을수록 우울증 경험자가 많았다.

 인제대 우 교수는 “어떤 우울증 약이라도 술보다 안정적이고 효과가 좋다”며 “우울증이 있는 사람은 전문의를 찾아 치료를 받아야 한다”고 말했다.
