Leaving the battlefield (KOR)

 New Covid-19 cases are increasing in all cities and municipalities across the nation. Jung Eun-kyeong, head of the Korea Centers for Disease Control and Prevention (KCDC), warned that we have not reached the peak of the pandemic yet. To overcome an unprecedented crisis, the government must present accurate prescriptions to tackle the challenges, and citizens must comply with them faithfully and voluntarily. On top of that, the medical community’s dedicated participation in the battle against the virus is desperately needed, as proven during the first spike in Korea in February and March.
 
However, a number of doctors are leaving this challenging battlefield one after another in protest of government plans — including the establishment of a “public medical school” and a scheme to drastically increase the number of medical students. Following a one-day walkout by intern and resident physicians last Friday, their senior colleagues went on strike from Sunday and Monday, to be followed by an all-out labor action by medical practitioners in the country for three days from Wednesday. A critical medical vacuum in the face of a resurgence in Covid-19 infections rings loud alarms across the battlefield.
 
Some university medical centers have already announced they can hardly accept patients in need of emergency care under such circumstances. Other hospitals joined the chorus by demonstrating a reluctance to accept people who want to get coronavirus tests. In some cases, cancer operations have been delayed for several weeks. No wonder a growing number of people are worrying about the possibility of deaths among patients with diseases other than Covid-19.
 
The Korea Medical Association (KMA) of 130,000 doctors has urged the Moon Jae-in administration to change course after defining its schemes to raise admissions to medical colleges, set up what it calls a “public medical school,” apply insurance to oriental medicine and allow remote treatments. The KMA calls these the “four evils” of the administration’s medical policy. A recent meeting between the KMA head and health minister only confirmed their sharp disagreements. Despite the minister’s pledge to put the four controversial plans on hold until Covid-19 cases ease, the KMA has threatened to take action unless the government withdraws the plans. The game of chicken must end.
 
Despite a meeting on Monday between KMA excecutives and Prime Minister Chung Sye-kyun, no tangible results were announced. The Korean Intern Resident Association (KIRA) said its members would not return to emergency rooms and ICUs except when related to Covid-19 treatment. As the Covid-19 situation has reached a new level of danger, the government and medical community must avert a possible collapse of our medical system. If the risk of seasonal flu is added to the Covid-19 crisis, the situation will truly get out of control. Even though they oppose government policy, doctors must return to their hospitals.
 


어떤 경우에도 응급실·중환자실 진료 공백은 안 된다
 
코로나19 확진자가 전국 17개 시·도에서 동시다발적으로 쏟아져 나오고 있다. 아직 정점이 아니라는 정은경 질병관리본부장의 경고처럼 2차 대유행은 끝을 알 수 없는 상황이다.
 
당면한 위기를 극복하려면 정부의 정확한 정책 방향 제시, 국민의 자발적 방역 동참이 기본이다. 2~3월 1차 대유행 때 확인한 것처럼 의료계의 헌신적 참여도 필수불가결한 요소다.
 
그런데 지금 의사들이 현장을 떠나고 있다. 지난 21일 전공의(인턴·레지던트)들이 집단 휴진(파업)에 들어가더니, 23일부터는 레지던트 1~2년 차까지 가세했다. 24일부터는 전임의들이 파업에 돌입했고, 26일부터는 개업의들이 파업에 나선다.  
 
2차 대유행 사태를 총력 대응해도 부족한데 의사들의 파업까지 겹쳐 우려하던 의료 공백이 현실화하고 있다. 전공의·전임의·개업의의 전면 파업이 장기화할 경우 코로나19에 대한 체계적 대응이 어려워지고 일반 환자는 물론 응급환자와 중환자의 적기 진료에도 큰 차질이 우려된다.  
 
실제로 서울 시내 일부 대학병원의 경우 당분간 응급실로 오는 중환자를 받을 수 없다고 공지했다. 코로나 검사 희망자는 안 받겠다는 병원도 나왔다. 심지어 암 수술 일정이 몇 주 연기되는 사례도 나오면서 환자와 가족의 불만과 불안감이 커지고 있다. 이러다 코로나19가 아닌 다른 질병으로 먼저 죽겠다는 하소연이 나올 지경이다.
 
의사 13만 명으로 조직된 대한의사협회는 문재인 정부의 의대 정원 확대, 공공 의대 신설, 한방 첩약 급여화, 비대면 진료 도입을 '4대 악 의료정책'으로 규정하고 정책 철회를 촉구해왔다. 최대집 의협 회장은 지난 19일 박능후 보건복지부 장관을 만나 대화했지만, 이견만 드러내고 돌아섰다.  
 
이후 복지부는 수도권의 코로나19 상황이 안정된 이후 의료계와 논의해 추진한다는 유보 입장을 보였지만, 의협은 정책을 철회하지 않으면 집단행동을 예정대로 하겠다는 입장이다. 유보와 철회가 '치킨게임' 중인 셈이다
 
의협 집행부가 어제 정세균 총리를 만났지만, 대화 입장만 확인했을 뿐 구체적인 성과는 없었다. 대한전공의협의회는 코로나19 대응 외에 병동·응급실·중환자실 복귀는 없다는 입장이다. 누적된 불신과 갈등을 일거에 풀 수 없다면 계속 만나서 절충점을 반드시 찾아야 한다.  
 
감염병 전문가들이 거리두기 3단계 격상을 촉구할 정도로 상황이 급박하다. 최악의 의료 시스템 붕괴 사태는 막아야 한다. 자칫 위기 대응 타이밍을 놓쳐 환절기 독감 환자까지 가세하면 코로나19 대응은 이중 삼중으로 어려움이 예상된다.  
 
백신도 치료제도 아직 기약이 없는 상황에서 국민이 기댈 곳은 의사와 병원뿐이다. 정부 정책에 반대하더라도 국민의 생명과 안전을 볼모로 한 파업은 명분이 없다. 의사들은 환자 곁으로 속히 돌아가야 한다.
